Adopter agrees to adopt the animal described above according to the terms and
conditions set forth herein:
-The Bird you are adopting has been in some way displaced. It may have been
neglected or abused. There may only be limited, if any, information available
regarding the birds’ previous environment. All available information has been
provided to you. DF&F!RS does not guarantee, nor is it responsible for the
temperament or health of this bird, other than is separately set forth in this
Contract. There will be an adjustment period. Remember to be patient, consistent
and understanding.
Please read all and initial when done.
1. Adopter agrees to provide this Bird with proper food, water, and loving attention.

2. Adopter agrees to keep this Animal in a large cage/loft and to provide Animal with
adequate toys and perches. __________
3. Adopter agrees to obtain pertinent information regarding the proper care of this Bird and to seek assistance from appropriate sources including DF&F!RS, as needed.

4. Adopter understands that DF&F!RS is not responsible for any incidence of disease in the adopted bird detected or diagnosed more than five (5) days after the adoption. ________
5. Adopter understands that it is his/her responsibility to quarantine any adopted bird brought into his/her home for not less than 30 days if the adopter has other birds__________
6. Adopter shall allow DF&F!RS to inquire about this bird at any reasonable time. Adopter
also must share pictures of the adopted bird/birds via text or on DF&F! FB page from time to time or when asked ____________
7. Adopter agrees that he/she will provide the best possible care for their adopted bird.

8. If for ANY reason he/she is unable to keep this bird he/she agrees to relinquish this bird to DF&F!RS without monetary exchange. DF&F!RS has the right to pursue legal action if bird is rehomed without permission.______
9. Adopter is aware that these birds were socialized to people or ex-sportsman’s birds. They are imprinted and CANNOT under any circumstances be released in to the
wild.__________
10. Adopter agrees not to free fly this bird, race, or use for training of hunting dogs or shooting events._______
11. I/we affirm that no member of my household has been convicted of an animal welfare law violation such as neglect, cruelty, abandonment etc_______
12. Adopter agrees to accept this Bird as a companion Bird to live in Adopter’s home or
approved loft. ______
RELEASE
- I/we the undersigned, acknowledge that there are risks with handling/living with
exotic birds. These include but are not limited to: Illness or injury to pets or
myself and/or damage to my property. _______
- I/we agree to release Dori’s Flock & Friends! Rescue/Sanctuary from any
liability in the event of illness, injury, legal issues, fines, or property damage
which occurs as a result of my exposure to any DF&F!R/S birds. _______
- I, ________________________________________, hereby agree that the above
described bird(s) is/are being adopted by me solely as a pet for myself and/or my
immediate family. I agree that I will not race, release, sell, give away or otherwise
dispose of said animal to any person(s), dealer, retailer, auction, institute or any
other entity for any reason. DF&F!R/S must be contacted if pets are to be
rehomed.
I have read this Release and understand its terms. I acknowledge that this
Release is legally binding, and agree to be legally bound by its provisions. I have
received a copy of this contract. All adult members in household where bird will
reside MUST sign this contract.
